This form is for refill of medication only. You must be a current patient of Worthington Family Physicians. Your prescription will be called in by 6pm (assuming you sent the form during our “on-line” hours: 8:30am to 4:30pm MON-TUE-WED-THU (Friday on-line hours are closed). Any form sent after 4pm on the above mentioned days will be considered the next business day. Please call your pharmacy before picking up your prescription to ensure it is there.

E-mail responses can take up-to 24 hours or longer if we would receive your on-line request form on a Tuesday or during a weekend.
