This form is for refill of medication only. You must be a current patient of Worthington Family Physicians. Your prescription will be called in by 6pm (assuming you sent the form during our “On-Line” hours: 8:30am to 4:30pm MON-THU). Any form sent after 4:30pm will be considered the next business day. Any form sent after 4:30pm between Thursday and Sunday will be replied to on Monday. Please call your pharmacy before picking up your prescription to ensure it is there.

E-mail responses can take up-to 24 hours or longer if we would receive your on-line request form on a Tuesday or during a weekend.
